6th Annual CF Physical Therapy-Jill Bane Memorial Scholarship

CF Physical Therapy, along with the family of the late Jill Bane, is co-sponsoring the 6th Annual CF Physical Therapy-Jill Bane Memorial Scholarship for area high school students planning on entering the field of physical therapy or athletic training.

New in 2016, an undergraduate college student or post-graduate student currently studying physical therapy or athletic training is also eligible to apply for one of the three $1000 awards.

Jill Bane was well-known for serving the Pontiac and Livingston county areas as an Athletic Trainer and Physical Therapist Assistant for CF Physical Therapy for over 10 years. Highly regarded by the athletes, parents, and patients she worked with, she served as a mentor and inspiration to fellow employees and students interested in the athletic training and physical therapy fields before losing her courageous battle with lymphoma in 2009. In the spirit of her commitment to advancing patient care, CF Physical Therapy, in conjunction with Jill’s family, began this scholarship program.

This scholarship, first awarded in 2010, is available to any high school senior who plans to pursue a career in either physical therapy or athletic training and resides within a thirty-mile radius of any of the twenty-two CF Physical Therapy clinics in central and southeastern Illinois and who has maintained a minimum 3.0/4.0 or 4.0/5.0 GPA. Beginning in 2016, this scholarship is also available to any undergraduate student or post-graduate student currently enrolled in a physical therapy or athletic training field of study whose permanent residence is within a thirty-mile radius of any of the twenty-two CF Physical Therapy clinics in central and southeastern Illinois and who has maintained a minimum 3.0 GPA.

The following criteria are also required for consideration from ALL applicants:

• A valid email address

• Proof of permanent address

• One letter of character reference from a non-family member

• A copy of the student’s most recent transcript (inc. GPA)

• A 600-word maximum essay, double-spaced, discussing the following prompt-“Discuss the three most meaningful experiences that led you to choose a career in physical therapy or athletic training.”
